The app keeps a local SQLite database as the source of truth and mirrors everything to a cloud database. On startup it intelligently reconciles the two — pushing local changes up or pulling cloud changes down based on which is ahead. Patient IDs are generated by cross-checking a persisted counter file and the highest ID in the database, then taking whichever is greater. This guards against gaps and collisions even if the app is used on more than one workstation or restored from a backup.
